1095 Reports: How to Request Reports for Former Employees with COBRA Benefits
Employer groups that administer their COBRA benefits through Blue Cross and Blue Shield of Texas can log in to HealthEquity (formerly WageWorks) and request a 1095 report for those employees.
Steps to request a 1095 report:
Log in to the HealthEquity portal .
On the main page, click on the grey tab labeled Cases.
In the top left-hand corner, choose Create a New Case.
What can we help you with today? Choose COBRA from the drop-down menu.
How would you categorize your question? Choose General Request.
In the Request Details field, enter a brief description that states you need a 1095 report and the year.
Next, choose Proceed to Submit Request.
Add any attachments if needed.
Choose Submit Request.
The report will be emailed to the requester.
Background: The 1095 tax form reports information about an employee's health coverage.
