All ID cards display participants%u2019 ID numbers, plan names, deductible and maximum out-of-pocket information and coverage start dates. If a participant%u2019s plan requires a PCP, their doctor%u2019s name is on the ID card. Participants can also find copay information and the phone number for PHGs. Note: TRS-ActiveCare participants who change plans and newly enrolled TRS-ActiveCare participants will receive new ID cards for the plan year.
